$500 for a OHHS graduating student going into Sports Medicine or related field

● No GPA minimum
● High School Transcript
● List of school activities, community service, honors, awards and work experience
● 1 letter of recommendation from anyone who knows your potential. Must include the reference’s name, address and phone number.
● Write an essay briefly describing the area of sports medicine/training you wish to pursue and the educational path you intend to follow. Include how you intend to empower athletes in your career; and what will you do to promote athletic safety.
